Supported Living
Independence, Supported Every Step of the Way
Your home. Your tenancy. Your life. We provide the right level of skilled, person-centred support for adults with learning disabilities, autism, and complex needs to live independently and thrive.
What We Offer
What Is Supported Living?
Supported living is a model of care that enables adults with learning disabilities, autism spectrum conditions, acquired brain injuries, mental health conditions, and other complex needs to live in their own home — with their own tenancy — and receive the individual support they need to do so successfully. This is fundamentally different from a care home or residential setting: in supported living, you are a tenant in your own right, not a resident in someone else's service.
The support we provide focuses on outcomes, not just tasks. Rather than doing things for people, we work alongside individuals to build skills, grow confidence, and develop independence over time. The intensity of support can range from a few hours per week for someone who needs help managing their budget and accessing the community, to near-constant 24/7 support for those with higher or more complex needs. Our nurse-led team ensures clinical and safeguarding standards are maintained across every package, while always keeping the individual — and their goals — at the absolute centre of everything we do.
What's Included
How We Support Independent Living
Our supported living packages are built around what matters to each individual — not a standard list of tasks. Here's the range of areas our support workers specialise in.
Daily Living Skills Development
Cooking, cleaning, personal hygiene, household routines — we support individuals to learn and develop these skills at their own pace, building real confidence and lasting independence.
Personalised Support Planning
Co-produced support plans that reflect individual goals, communication preferences, risk assessments, and personal outcomes — reviewed regularly and updated as circumstances change.
Community & Social Participation
Supporting individuals to access leisure activities, community events, friendships, and social networks — reducing isolation and building a fulfilling life beyond the front door.
Employment, Education & Volunteering
Practical support with job searching, college enrolment, volunteering opportunities, travel training, and preparing for interviews — helping individuals pursue their own ambitions and aspirations.
Budget & Tenancy Management
Support with managing personal finances, understanding bills and benefits, maintaining a tenancy, and navigating interactions with landlords and statutory services — building long-term stability.
Mental Health & Emotional Wellbeing
Consistent emotional support, help with anxiety management strategies, liaison with mental health services, and a stable, trusting relationship with a familiar support worker who knows the individual well.
Who It's For
Who We Support
Our supported living service is designed for adults aged 18 and over who want to live independently with the right level of tailored support around them. We work with people across a wide range of needs.
Learning Disabilities
Individualised, respectful support for adults with mild, moderate, or severe learning disabilities — promoting autonomy, dignity, and meaningful participation in community life.
Autism Spectrum Conditions
Support workers trained in autism-informed practice, understanding sensory needs, communication styles, and the importance of predictable routines and consistent relationships.
Acquired Brain Injuries
Skilled, patient support for adults who have experienced a brain injury and are rebuilding independence — adapting to changing cognitive and physical abilities over time.
Mental Health Conditions
Supportive, recovery-focused assistance for adults managing mental health conditions who want to live independently — with consistent support that doesn't disappear at the first sign of stability.
Sensory Impairments
Support adapted to the specific requirements of those with visual or hearing impairments, using appropriate communication methods and assistive technology.
Complex or Multiple Needs
For individuals with multiple conditions requiring a co-ordinated, specialist team approach — our nurse-led oversight ensures complex needs are managed safely and holistically.
Getting Started
How It Works
We work closely with individuals, their families, and local authority commissioners to get supported living right from the start.
Free Consultation
Contact us to discuss the individual's needs, current situation, and goals. We welcome calls from individuals themselves, family members, social workers, and local authority care managers.
Needs Assessment
We carry out a thorough, person-centred assessment — listening to what matters most to the individual — and co-produce a detailed support plan that reflects their goals, preferences, and risk profile.
Support Begins
We introduce a consistent support worker, begin the package with a careful transition, and hold regular review meetings to ensure the support continues to work — and to celebrate progress made.
Why Choose Us
Why S&N Flexi Agency for Supported Living?
We believe every person with a disability deserves support that genuinely enables them to live the life they choose. Here's how we deliver on that promise.
Outcomes-Driven Approach
We measure success by the real-life outcomes individuals achieve — greater independence, new skills, stronger relationships, meaningful employment — not by the number of tasks ticked off a rota.
Co-Produced Support Plans
Support plans are written with the individual — not about them. We use accessible formats, support communication in whatever form works best, and ensure the person's own voice drives every decision.
Experienced Specialist Staff
Our support workers are selected for their values as much as their skills. They receive specialist training in learning disabilities, autism, positive behaviour support, and safeguarding, with ongoing clinical supervision.
Strong Local Commissioning Relationships
We work closely with Bolton Council and Cumberland Council, as well as NHS and independent commissioners, to ensure funded packages are arranged efficiently and that individuals receive the support they are entitled to.
Common Questions
Supported Living — FAQ
Have more questions? Call us on 01204 952 009 or speak to your social worker about making a referral.
What is the difference between supported living and residential care?
In residential care, you live in a provider-owned building and share it with other residents. In supported living, you have your own tenancy — in your own flat or shared house — and a separate support provider delivers care and support in that space. This separation gives you greater legal rights, more control over your living situation, and the ability to change your support provider without having to move home. It is widely regarded as best practice for adults with learning disabilities and autism who have the capacity and desire to live in the community.
Who funds supported living?
Supported living is most commonly funded by local authority social services following a Care Act assessment, sometimes in combination with NHS Continuing Healthcare funding or Personal Independence Payments. Housing costs are usually covered separately through housing benefit or personal rental payments. We work with social workers and local authority commissioners at both Bolton and Cumberland Councils and can advise on the funding pathway. Some individuals also self-fund or use a Direct Payment to commission support independently.
Can support levels change over time?
Absolutely — and we actively plan for this. As individuals develop skills and independence, it may be appropriate to reduce support hours over time. Equally, if needs increase due to changes in health, wellbeing, or circumstances, we can scale up the package accordingly. We carry out regular formal reviews of every support plan and maintain close communication with commissioners to ensure funding reflects actual need at all times.
Do you work with local authorities in Bolton and Cumbria?
Yes. We are an established provider with strong relationships with social work teams and commissioners at Bolton Council and Cumberland Council, as well as wider NHS referral routes. Our 91% RAAG Green Rating from Cumberland Council reflects the quality and reliability of our service. If you are a social worker or commissioner looking to make a referral or discuss a potential package, please contact us directly.
What conditions do you support?
We specialise in supporting adults with learning disabilities, autism spectrum conditions, acquired brain injuries, and mental health conditions. We also support individuals with sensory impairments and those with complex or multiple needs requiring a co-ordinated approach. Our nurse-led team, led by Susan Olueh (Registered Learning Disabilities Nurse), brings specific clinical expertise to complex cases. If you are unsure whether we are the right provider for a particular individual, please call us — we are always happy to discuss.
