What Is Outreach & Community Support?
Outreach support extends professional care beyond the home and into the wider community. While homecare focuses on supporting you within your own four walls, outreach is about everything else — the appointments you need to attend, the shops you want to visit, the social groups you'd like to join, and the opportunities you deserve to pursue. It's care that looks outward, not inward, with a genuine focus on participation, integration, and independence.
Our outreach and community support workers are skilled at enabling people to access and navigate everyday life safely and confidently. Whether that means accompanying someone to a hospital appointment, supporting an autistic adult to use public transport independently, helping someone with a learning disability access a local gym or arts group, or supporting a person recovering from a mental health episode to gradually rebuild their social world — we work at the individual's pace, towards goals they have chosen for themselves. This service can stand alone or work alongside our homecare or supported living packages for a joined-up approach.

What areas do you cover for outreach support?
We primarily deliver outreach support across Bolton and the Greater Manchester area, and throughout Cumbria including Carlisle, Barrow-in-Furness, Kendal, and surrounding towns. We can often accommodate requests slightly outside these areas — particularly for planned activities further afield. Please contact us directly if you are unsure whether we cover your location, and we will do our best to find a solution.
How many hours per week can I access outreach support?
There is no fixed minimum or maximum. Some individuals access a couple of hours per week for a specific purpose, such as attending a weekly social group or a regular medical appointment. Others have more intensive packages running across multiple days. The level of outreach support is determined entirely by your assessed needs, your personal goals, and available funding. We'll work with you to find the right level — and review it regularly.
Can outreach support be combined with homecare visits?
Yes — and this is often the most effective approach. Many of our clients receive a combination of homecare visits for personal care and domestic support, alongside outreach hours for community access and social participation. Having one provider manage both parts of your package means better coordination, a more consistent relationship with your support workers, and a joined-up care plan that addresses both your home and community needs. We can discuss a blended package during your free assessment.
Is outreach support funded by social services?
Outreach and community support can be funded through a local authority social care package following a Care Act needs assessment, via a Personal Independence Payment (PIP) or Direct Payment that you manage yourself, or through NHS Continuing Healthcare. Some individuals also self-fund. We are experienced in working with both Bolton Council and Cumberland Council commissioners and can support you in understanding and accessing available funding. Please don't let uncertainty about funding put you off making an initial enquiry — we can help you navigate the options.
How is safety managed during community outings?
Safety is central to every outreach package. Before any community activity begins, we carry out individual risk assessments covering the specific environments to be visited, the individual's known triggers or vulnerabilities, transport arrangements, and emergency protocols. Our support workers are trained in first aid, positive behaviour support, and lone working procedures. All outings are documented and reviewed, and our on-call team is available to support workers in the field at any time. We never take unnecessary risks — but we also never let unnecessary caution prevent people from living full lives.
